§ 163.02. Work reports on employees.

At the end of each six (6) months' period, following the date of an original appointment, or oftener if required by the civil service commission, pursuant to the power vested in that commission by virtue of paragraph (n) of Section 149 of the Charter of the city, the head of the division, bureau or department, upon forms furnished by the civil service commission, shall submit a record of the efficiency, character and conduct of each employee under his supervision, and, after approval by the commission thereof, the rating shall be recorded on the service record of the employee.
